Rapid implementation and spread
On the emerging Hospital at Home type of care concept in Germany we partnered with a project consortium.
We set out to rapidly implement and dynamically scale their care model because especially for elderly inpatients that have a certain level of autonomy, the hospital is a more dangerous place compared to their homes. By focusing on the context and iterating adoption mechanics, we achieved a faster implementation with measurable performance indicators for continuous evaluation and adjustment by the clinic. This matters because a successful pilot shows feasibility, motivates stakeholders and eventually can be measured by the economic impact for consistent planning. Next, we’ll roll-out to further units for regional scaling and higher throughput. -

Sustainable (re-)uptake
In a large hospital in Denmark daily digitalization of processes is a common companion beside the normal routines. But the triviality sometimes is tricky.
We set out to restore and sustain clinical utilization of a novel documentation platform because the clinic reported low usage of certain device records that fell silent some time after installation. By focusing on journeys, workflows and interdisciplinary guidance for educational loops, we achieved the visualization of barriers and identified drivers that we integrated to a resilient learning framework. This matters because only little use in the routines beyond integration means coming short of the efficiency and quality that was intended to achieve by the initial “Why” this has been procured in the first place. Next, we’ll accompany the clinic and track the performance during different stress scenarios to facilitate a robust sustainable knowledge base. -
Success Scenario 3
De-risk implementation
During a cooperation with a regional clinic, a private hospital group, the following scenario loomed.
We set out to pilot the adoption of a novel intermediate ward monitoring for high maintenance patients to relieve staff because clinicians were under constant stress, with a danger of quitting, managing the transitions by working right at the capacity limits and variables were high if that digitalization would fit the needs in that environment. By focusing on key narratives and determinants we achieved staff satisfaction during the change as well as visibility of the adoption. This matters because confident decision-making can be done early. Next, we’ll together withdraw from the first piloting-site and scale it at a unit with better conditions focussing on a refined use case. -

Composite System Diagnostics
A highly innovative university hospital connected with outpatient care facilities was calling us after evaluating the first results of their digitalization strategy in contrast to their medical care strategy and the economical impact.
We set out to create transparency and accountability across complex AI-enabled healthcare workflows because efficiency that is often promised by AI frequently does not materialize once systems become interconnected through multiple interfaces, data sources, and stakeholders. By focusing on neutral end-to-end diagnosis, workflow visualization, and identification of data and interface bottlenecks, we achieved a clearer understanding of where breakdowns actually occur — whether due to poor data quality, implementation errors, incomplete capture, or distorted handoffs between systems. This matters because no single manufacturer takes ownership of the full conglomerate system, leaving responsibility unclear and issues unresolved — and INCH Health can fill this gap as an independent diagnostician enabling sustainable system-level improvement.
